From welcoming a new individual into an ICF to coordinating a transfer or discharge, QIDPs play a critical role in ensuring these transitions are thoughtful, well-coordinated, person-centered, and compliant with regulatory requirements. Join us for our next QIDP Peer Learning Network session as we take a closer look at the admission, discharge, and transfer processes within ICFs/IID. We'll explore the federal and state regulations governing these processes, required assessments and documentation, team responsibilities, notification requirements, and the steps necessary to support successful transitions.
Come ready to share your experiences, ask questions, and learn from fellow QIDPs as we work together to turn regulatory requirements into effective, person-centered practices!
